Intervensi Latihan Pernapasan Pursed Lip Breathing dan Batuk Efektif Pada Pasien dengan Ppok: Studi Kasus

ABSTRACT

 

Chronic obstructive pulmonary disease (COPD) is a disease characterized by progressive airflow limitation and progressive destruction of lung tissue. COPD risk factors are smoking and age. This study was conducted to determine the effect of pursed lip breathing and effective coughing on COPD patients. The patient in this case report is a 60-year-old man with health complaints in the form of a cough that is not accompanied by phlegm and shortness of breath that increases when the patient is active. Nursing problems in patients are ineffective airway clearance associated with accumulation of mucus and excess secretions characterized by ineffective coughing, excess sputum, rhonchi sounds in the vesicular area. The results obtained in this study using non-pharmacological intervention methods used for symptom management in patients with COPD, namely pursed lip breathing and coughing, are effective in reducing symptoms of shortness of breath, although the decrease is not significant. After administering the intervention the patient's mMRC scale was 3 and the change in the COPD Assessment Test (CAT) score was 20 which means Moderate, COPD interferes with the patient's daily activities, respiratory frequency decreases and oxygen saturation can be maintained. The effect of effective physiotherapy and coughing accompanied by breathing exercises shows an increase in quality of life and reduces the symptoms felt by the patient. This intervention can be an alternative option to be carried out independently at home without using costs.

ABSTRAK

 

Penyakit paru obstruktif kronik (PPOK) merupakan penyakit yang ditandai dengan keterbatasan aliran udara progresif hingga kerusakan jaringan paru. Faktor risiko PPOK yaitu merokok dan usia. Penelitian ini dilakukan untuk mengetahui pengaruh latihan pernapasan pursed lip breathing dan batuk efektif pada pasien PPOK. Pasien dalam laporan kasus ini adalah seorang laki-laki berusia 60 tahun dengan keluhan kesehatan berupa batuk yang tidak disertai dahak dan sesak yang bertambah ketika pasien beraktifitas. Masalah keperawatan pada pasien yaitu bersihan jalan napas tidak efektif berhubungan dengan penumpukan lendir dan sekresi berlebih ditandai dengan batuk tidak efektif, sputum berlebih, terdengar bunyi ronchi di area vesikuler. Hasil yang didapatkan pada penelitian ini dengan menggunakan metode intervensi non farmakologis yang digunakan untuk manajemen gejala pada pasien dengan PPOK yaitu latihan pernapasan pursed lip breathing dan batuk efektk mampu mengurangi gejala sesak meskipun penurunannya tidak signifikan. Setelah pemberian intervensi skala mMRC pasien yaitu 3 dan perubahan pada skor COPD Assessment Test (CAT) yaitu 20 yang artinya Sedang, PPOK menggangu aktivitas sehari-hari pasien,frekuensi napas menurun dan saturasi oksigen dapat dipertahankan. Efek fisioterapi dan batuk efektif yang disertai latihan pernapasan menunjukkan peningkatan kualitas hidup dan menurunkan gejala yang dirasakan pasien. Intervensi ini dapat menjadi salah satu pilihan alternatif untuk dilakukan secara mandiri di rumah tanpa menggunakan biaya.
