Analisis Efektivitas Penggunaan Breast Board sebagai Alternatif Immobilisasi pada Limfoma Colli dengan Teknik Three Dimensional Conformal Radiation Therapy (3d-Crt) di Rumah Sakit Mrccc Siloam Semanggi

ABSTRACT
Colli Lymphoma is a rare type of cancer that affects the immune system. Colli (Carcinoma of Lymphoid Origin II) is a special category in non-Hodgkin's lymphoma that has characteristics in the process of causation and response to treatment. One way to treat Colli Lymphoma is to use external radiation therapy known as Three Dimensional Conformal Radiation Therapy (3D-CRT), which provides a more accurate distribution of radiation doses to the target area and reduces exposure to healthy organs (Organs at Risk/OAR). This study examines the effectiveness of using a breast board as a tool to maintain the position of Colli Lymphoma patients during 3D-CRT therapy at MRCCC Siloam Semanggi Hospital. The methodology used is a descriptive qualitative approach, with data obtained through observation, interviews, documentation, and analysis from the Treatment Planning System (TPS) and On Board Imager (OBI). The findings of the study suggest that breast boards have several advantages, such as providing more space for radiation to the axilla and neck areas, but there are also disadvantages such as lack of optimal fixation to the head. The results of the dose analysis showed that the larynx was the OAR with the highest dose received, while the apex of the lung received the lowest average dose. During the 20-day therapy process, verification of the patient's position showed a shift that remained within the tolerance limit. Therefore, the use of breast boards in the treatment of Colli Lymphoma can be effective in 3D-CRT therapy, although adjustments are needed to ensure the stability of the patient's position.

ABSTRAK
Limfoma Colli adalah tipe kanker yang langka dan berdampak pada sistem imun. Colli (Carcinoma of Lymphoid Origin II) merupakan suatu kategori khusus di limfoma non-Hodgkin yang memiliki ciri khas dalam proses penyebab dan reaksi terhadap pengobatan. Salah satu cara untuk menangani Limfoma Colli adalah dengan menggunakan terapi radiasi eksternal yang dikenal sebagai Three Dimensional Conformal Radiation Therapy (3D-CRT), yang memberikan distribusi dosis radiasi yang lebih akurat pada area target dan mengurangi paparan pada organ sehat (Organ Yang Berisiko/OAR). Penelitian ini meneliti efektivitas penggunaan breast board sebagai alternatif immobilisasi untuk menjaga posisi pasien Limfoma Colli selama terapi 3D-CRT di Rumah Sakit MRCCC Siloam Semanggi. Metodologi yang digunakan adalah pendekatan kualitatif deskriptif, dengan data diperoleh melalui observasi, wawancara, dokumentasi, dan analisis dari Treatment Planning System (TPS) serta On Board Imager (OBI). Temuan penelitian menyarankan bahwa breast board memiliki beberapa keuntungan, seperti memberikan ruang lebih untuk penyinaran ke daerah axilla dan leher, namun ada juga kekurangan seperti kurangnya fiksasi optimal pada kepala. Hasil analisis dosis menunjukkan bahwa laring merupakan OAR dengan dosis tertinggi yang diterima, sementara puncak paru-paru mendapatkan dosis rata-rata terendah. Selama 20 hari proses terapi, verifikasi posisi pasien menunjukkan adanya pergeseran yang tetap dalam batas toleransi. Oleh karena itu, penggunaan breast board sebagai alternatif immobilisasi dalam penanganan Limfoma Colli bisa efektif dalam terapi 3D-CRT, meskipun perlu adanya penyesuaian untuk memastikan stabilitas posisi pasien.
