Analisis Rasio Monosit–High Density Lipoprotein (HDL) Pada Pasien Diabetes Melitus Tipe 2 Dengan Komplikasi Hipertensi Sekunder Dan Sindrom Koroner Akut

Diabetes melitus tipe 2 (DMT2) merupakan penyakit metabolik kronis yang ditandai hiperglikemia akibat gangguan sekresi dan resistensi insulin. DMT2 berhubungan erat dengan peningkatan risiko komplikasi kardiovaskular, termasuk hipertensi dan sindrom koroner akut (SKA). Rasio monosit terhadap high density lipoprotein (Monocyte-to-HDL Ratio/MHR) diusulkan sebagai biomarker inflamasi dan aterosklerosis. Penelitian ini bertujuan menganalisis rasio monosit–HDL pada pasien DMT2 dengan komplikasi hipertensi sekunder dan sindrom koroner akut. Penelitian ini menggunakan desain penelitian analitik observasional dengan rancangan cross sectional. Data penelitian diperoleh secara consecutive sampling dari rekam medis pasien diabetes melitus di RSUD (Rumah Sakit Umum Dareah) R.T Notopuro Sidoarjo. Analisa menggunakan uji Kolmogorov-Smirnov dan uji t-test independent untuk menganalisa peran rasio MHR pada penderita penderita DMT2 dengan komplikasi hipertensi sekunder dan Sindrom Koroner Akut. Sebanyak 31 subjek penelitian terdiri atas 27 pasien DMT2 dengan hipertensi sekunder dan 4 pasien DMT2 dengan SKA. Nilai rerata MHR pada kelompok hipertensi sekunder sebesar 0,022 ± 0,013, sedangkan pada kelompok SKA sebesar 0,018 ± 0,005. Tidak ditemukan perbedaan bermakna nilai MHR antara kedua kelompok (p=0,542). Analisis korelasi menunjukkan hubungan positif sangat kuat antara kadar monosit dan MHR (r=0,813; p<0,001), serta hubungan negatif bermakna antara kadar HDL dan MHR (r=-0,432; p=0,015). Tidak terdapat perbedaan signifikan nilai MHR antara pasien DMT2 dengan hipertensi sekunder dan pasien DMT2 dengan SKA. Kadar monosit berkorelasi positif kuat terhadap MHR, sedangkan kadar HDL berkorelasi negatif terhadap MHR. Penelitian lebih lanjut dengan jumlah sampel yang lebih besar dan desain prospektif diperlukan untuk mengevaluasi peran MHR sebagai biomarker komplikasi kardiovaskular pada DMT2
