Penatalaksanaan Sindrom Wolff–Parkinson–White Tipe B Pada Kehamilan Trimester Ketiga: Laporan Kasus Dan Tinjauan Literatur

Sindrom Wolff–Parkinson–White (WPW) merupakan kelainan pre-eksitasi kongenital yang dapat memicu takikardia supraventrikular. Perubahan hemodinamik, hormonal, dan otonom selama kehamilan meningkatkan risiko aritmia sehingga penatalaksanaannya harus mempertimbangkan keselamatan maternal dan fetal. Dilaporkan seorang perempuan berusia 33 tahun, gravida 2 para 1, dengan usia kehamilan 37 minggu yang mengalami keluhan berdebar hilang timbul. Dua bulan sebelumnya, pasien mengalami takikardia supraventrikular dengan aberansi yang berhenti setelah pemberian propafenon oral. Elektrokardiografi menunjukkan interval PR memendek, kompleks QRS melebar, gelombang delta, dan gelombang S dominan pada sadapan V1, konsisten dengan WPW tipe B. Ekokardiografi menunjukkan struktur dan fungsi jantung normal. Karena kondisi hemodinamik stabil, episode aritmia jarang, struktur jantung normal, dan respons terapi sebelumnya baik, dipilih propafenon oral 300 mg dengan strategi pill-in-the-pocket. Strategi ini dipilih untuk mengendalikan konduksi melalui jalur aksesori sekaligus meminimalkan paparan obat kontinu terhadap janin. Adenosin tidak diberikan karena tidak terdapat episode takikardia akut saat evaluasi. Pasien menjalani persalinan pervaginam tanpa komplikasi maternal maupun fetal dan bayi lahir dalam kondisi baik. Studi elektrofisiologi dan ablasi kateter direncanakan pascapersalinan. Kebaruan kasus ini terletak pada keberhasilan penggunaan propafenon intermiten pada WPW tipe B di akhir kehamilan. Pendekatan konservatif dengan pemantauan ketat dapat menjadi pilihan pada pasien terpilih dengan hemodinamik stabil.
