Hubungan Kadar Insulin Puasa dengan Indeks Massa Tubuh Pada Kelompok Lanjut Usia di Panti Lanjut Usia

ABSTRACT

 

Elderly population is increasing globally. One of the global adversity is obesity. Prevalence of obesity in the elderly population is growing over time and has become a global threat. Obesity also affect insulin level and resistancy, becoming the foundation for diabetes melitus, the unending global health problem. This will bring various health risks for elderly. This study aims to find out the correlation between fasting insulin level with body mass index in the elderly population. This cross-sectional study was done in Santa Anna Nursing Home. The sampling method is total sampling. Body mass index data collection was carried out according to standard physical examination protocols and insulin level analysis was carried out according to standard laboratory protocols. Statistic analysis used in the study is Spearman correlation, with hoped significance level of 5% and power of 80%. We found 31 respondents with mean age of 73.1 (±9.55) years with female as the dominant gender. The median of insulin level were 10 (5,4 – 29,,5) uIU/mL, with mean BMI of 22,24 (3,95) kg/m2. Statistical analysis found no significant correlation between fasting insulin level with BMI (p-value : 0.179), but clinically could be seen that increase of fasting insulin level will affect the increase of BMI though its only a weak correlation (r-spearman : 0.248). Fasting insulin level plays a role and could affect body mass index in the elderly body.

ABSTRAK

 

Jumlah penduduk lanjut usia secara global akan terus meningkat. Salah satu permasalahan yang mendunia adalah obesitas. Prevalensi obesitas pada populasi lanjut usia kian meningkat dan menjadi ancaman global. Obesitas juga memengaruhi kadar dan resistensi insulin yang menyebabkan diabetes melitus menjadi masalah kesehatan yang tak kunjung berakhir di dunia. Hal ini menjadi dasar berbagai risiko kesehatan pada lansia. Studi ini dilakukan untuk mengetahui apakah terdapat hubungan yang bermakna antara kadar insulin puasa dengan indeks massa tubuh pada kelompok lanjut usia. Studi cross-sectional ini dilaksanakan di Panti Lanjut Usia Santa Anna. Metode pengambilan sampel berupa total sampling. Pengambilan data indeks massa tubuh dilakukan sesuai protokol pemeriksaan fisik standar dan analisa kadar insulin dilaksanakan sesuai protokol laboratorium standar. Analisa statistik yang digunakan pada penelitian ini berupa analisa korelasi spearman, dengan nilai signifikansi yang diharapkan pada penelitian ini adalah sebesar 5% dengan kekuatan penelitian sebesar 80%.  Didapatkan 31 responden dengan rerata usia sebesar 73,1 (±9,55) tahun dengan dominan berjenis kelamin perempuan (71,0%). Rerata kadar insulin puasa adalah 10 (5,4 – 29,,5) uIU/mL, dengan rerata IMT 22,24 (3,95) kg/m2. Pada uji statistik tidak didapatkan korelasi yang bermakna antara kadar insulin puasa dengan IMT (p-value : 0.179), tetapi secara klinis dapat terlihat bahwa peningkatan kadar insulin puasa akan berdampak terhadap peningkatan IMT walaupun tergolong dalam korelasi lemah (r-spearman : 0.248). Kadar insulin puasa tampak memegang peranan dan dapat berdampak terhadap indeks massa tubuh pada lansia.
