Gambaran Kelelahan pada Klien dan Keluarga dengan Diabetes Melitus

ABSTRACT
Diabetes Mellitus (DM) type 2 occupies 90% of all cases of diabetes and causes symptoms of polyphagia, polydipsia, polyuria, weight loss and fatigue. Fatigue related DM is associated with physical and psychological fatigue which is most commonly reported in chronic disease conditions. Cellular starvation in DM is a starvation condition experienced by cells because glucose is difficult to enter cells. This fatigue condition impacted in a long-term DM treatment and DM control. Nurses need to identify and evaluate the level of fatigue by using a valid and reliable instrument. This research is descriptive with a sample of 26 DM clients and DM family caregiver. The instrument used in this study is the Multidimensional Fatigue Inventory (MFI) which includes 20 question items with a Likert scale. The results showed that 54% of respondents experienced low fatigue and reported moderate fatigue as much as 31%. This shows that DM clients and families who care for DM clients have a level of fatigue that must be monitored and given comprehensive treatment. It is recommended for further researchers to identify the factors that influence DM client fatigue, so that appropriate and accurate self-management interventions can be given. Proper handling of fatigue can minimize other nursing problems such as activity intolerance and nursing care dependency.

ABSTRAK
Diabetes Melitus (DM) tipe 2 menempati 90% dari keseluruhan kasus diabetes dan menimbulkan gejala polyphagia, polidipsi, polyuria, penurunan berat badan dan kelelahan. Kelelahan pada DM dikaitkan dengan kelelahan fisik dan psikis yang paling banyak dilaporkan pada kondisi penyakit kronis. Starvasi seluler pada DM merupakan kondisi kelaparan yang dialami oleh sel karena glukosa sulit masuk kedalam sel. Kondisi kelelahan ini menyebabkan perawatan jangka Panjang DM dan pengendalian DM yang memberikan dampak pada keluarga yang merawat. Perawat perlu mengidentifikasi dan mengevaluasi tingkat kelelahan dengan menggunakan instrument yang valid dan reliabel. Penelitian ini adalah deskriptif dengan jumlah sampel 26 orang klien DM dan keluarga klien DM.Instrumen yang digunakan dalam penelitian ini adalah Multidimensional Fatigue Inventory (MFI) yang meliputi 20 item pertanyaan dengan skala likert. Hasil penelitian menunjukkan 54 % responden mengalami kelelaran rendah dan melaporkan kelelahan sedang sebanyak 31%. Hal ini menunjukkan bahwa klien DM dan keluarga yang merawat klien DM memiliki tingkat kelelahan yang harus dimonitor dan diberikan penanganan yang komprehensif. Disarankan bagi peneliti selanjutnya untuk mengidentifikasi faktor-faktor yang berpengaruh terhadap kelelahan klien DM, sehingga dapat diberikan intervensi self manajemen secara tepat dan akurat. Penanganan kelelahan yang tepat dapat meminimalisir masalah keperawatan yang lain seperti intoleransi aktivitas dan ketergantungan keperawatan.
