Hubungan Status Stoma dengan Tingkat Kecemasan pada Pasien Tumor Ganas Kolorektal
Abstract
Kanker kolorektal merupakan salah satu keganasan dengan angka kejadian yang terus meningkat secara global. Salah satu tatalaksana pembedahannya adalah pembuatan stoma, baik sementara maupun permanen, yang berpotensi menimbulkan dampak psikologis, termasuk kecemasan. Penelitian ini bertujuan untuk mengetahui hubungan antara status stoma dengan tingkat kecemasan pada pasien tumor ganas kolorektal di RSUD Dr. H. Abdul Moeloek periode 2021–2025. Penelitian ini merupakan penelitian deskriptif analitik dengan pendekatan kuantitatif menggunakan desain cross-sectional. Populasi penelitian terdiri atas seluruh pasien tumor ganas kolorektal yang menjalani pemakaian stoma di RSUD Dr. H. Abdul Moeloek periode 2021–2025 sebanyak 40 orang. Teknik pengambilan sampel yang digunakan adalah total sampling, sehingga seluruh anggota populasi yang memenuhi kriteria penelitian dijadikan responden. Tingkat kecemasan diukur menggunakan Hamilton Anxiety Rating Scale (HARS) yang diisi langsung oleh responden pada periode 16 Februari hingga 16 Maret 2026 setelah menjalani pemakaian stoma, sedangkan status stoma diperoleh melalui data rekam medis. Data dianalisis menggunakan uji chi-square dengan tingkat kepercayaan 95% dan α = 0,05. Pada kelompok stoma permanen, kecemasan berat ditemukan pada 61,5% responden, lebih tinggi dibandingkan kelompok stoma sementara sebesar 25,9%. Uji chi-square menunjukkan adanya hubungan yang signifikan secara statistik antara status stoma dengan tingkat kecemasan (p = 0,032). Terdapat hubungan yang bermakna antara status stoma dengan tingkat kecemasan pada pasien tumor ganas kolorektal, dengan pasien yang memiliki stoma permanen cenderung mengalami kecemasan yang lebih berat. Keterbatasan penelitian ini meliputi jumlah sampel yang relatif terbatas, kondisi fisik sebagian responden yang kurang optimal, serta variabel penelitian yang belum mencakup seluruh faktor yang dapat memengaruhi tingkat kecemasan. Hasil penelitian ini menekankan pentingnya skrining kesehatan mental dan intervensi psikologis yang terintegrasi dalam penatalaksanaan pasien kanker kolorektal.
