Abstract

Background: Low Birth Weight (LBW) (<2,500 grams) is a primary contributor to neonatal mortality. In 2025, Cengkareng Primary Health Center recorded 52 LBW cases out of 12,527 live births. Methods: This descriptive observational study utilized a retrospective approach analyzing secondary LBW data at Cengkareng Health Center in 2025. Risk factors were examined using a fishbone diagram, followed by priority problem selection via the Urgency, Seriousness, Growth (USG) method, and intervention planning using Planning, Organizing, Actuating, Controlling (POAC) framework. Results: Fishbone analysis revealed multifactorial determinants: low maternal nutritional knowledge, chronic energy deficiency, anemia, poor iron supplement compliance, irregular Antenatal Care (ANC) visits, inaccurate weighing scales, and lack of family support. USG prioritization identified inadequate nutritional intake, non-routine ANC visits, and micronutrient deficiencies as the primary issues. Recommended POAC interventions include enhanced health education, Pregnant Women’s Classes, Husband Classes, P4K empowerment, and digital monitoring via the "SIGAP BUNDA" WhatsApp group. Conclusion: Mitigating LBW requires a comprehensive approach integrating maternal nutrition education, strict ANC compliance, digital health monitoring, and strong family involvement supported by healthcare providers.
