Penerapan Metode SCREEM pada Program Skrining Penyakit Tidak Menular Puskesmas Nanjungmekar dalam Mengevaluasi Ketidakpatuhan Minum Obat Hipertensi

Penyakit Tidak Menular (PTM) merupakan salah satu penyebab utama kematian di Indonesia, dengan hipertensimenjadi salah satu faktor risiko utama yang perlu diatasi. Penelitian ini bertujuan untuk mengevaluasi ketidakpatuhan pasien hipertensi dalam menjalani pengobatan di Puskesmas Nanjungmekar, menggunakan metode SCREEM untuk mengidentifikasi faktor- faktor biopsikososial yang berperan. Dalam kegiatan ini, 53 warga Desa Nanjungmekar menjalani skrining, dan terdapat 28 individu dengan hipertensi. Metode wawancara dilakukanterhadap 20 individu dengan hipertensi untuk mengumpulkan data. Dari 28 individu, 20 orang (71,4%) tidak patuh terhadap pengobatan. Hasil wawancara menunjukkan bahwa lima aspek dominan—sosial (75%), pendidikan (55%), budaya (20%), ekonomi (15%), dan religius (10%)—mempengaruhi ketidakpatuhan pengobatan hipertensi. Aspek medis tidak ditemukan memberikan kontribusi signifikan terhadap ketidakpatuhan pengobatan pada kelompok ini.Penerapan metode SCREEM memberikan informasi spesifik terkait faktor-faktor yang perlu diperhatikan untuk meningkatkan kepatuhan terhadap pengobatan hipertensi melalui pendekatan yang lebih komprehensif dalam layanan kesehatan masyarakat. Luaran dari penerapan metode SCREEM ini dapat menjadi dasar rekomendasi untuk merancang program yang berfokus pada penguatan aspek sosial dan pendidikan dalam mendukung kepatuhan pengobatan hipertensi di Puskesmas Nanjungmekar.

ABSTRACT
Non-communicable diseases (NCDs) are one of the leading causes of death in Indonesia, with hypertension being oneof the main risk factors that need to be managed. This study aimed to evaluate hypertension patients' non-compliance with treatment at Puskesmas Nanjungmekar, using the SCREEM method to identify biopsychosocial factors at play. In this study, 53 individuals from Nanjungmekar village were screened, and 28 individuals with hypertension were found. An interview method was conducted with 20 individuals with hypertension to gather data. Of the 28 individuals, 20 (71.4%) were not compliant with their medication. The results showed that five dominant aspects—social (75%), educational (55%), cultural (20%), economic (15%), and religious (10%)—affecthypertension treatment non- compliance. Medical aspects were not found to contribute significantly to treatment non-compliance in this group. The application of the SCREEM method provides specific information regardingfactors that need to be considered to improve adherence to hypertension treatment through a more comprehensive approach in public health services. The outcomes of the application of the SCREEM method can be the basis of recommendations for designing programs that focus on strengthening social and educational aspects in supporting hypertension treatment compliance at the Puskesmas Nanjungmekar.
