Deteksi Dini Kesehatan Masyarakat: Skrining Penyakit Kardiometabolik Masyarakat Kelurahan Situsaeur

ABSTRAK

 

Beban penyakit kronis menyebabkan hilangnya masa hidup yang sehat selama hampir 7 tahun. Adanya kondisi disabilitas karena faktor risiko tidak dikelola dan diidentifikasi dari awal membuat beban penyakit di Indonesia semakin besar khususnya penyakit yang masuk dalam cakupan kardio-renal-metabolik. Kondisi ini juga diperberat dengan penurunan fungsi ginjal yang terjadi dalam tingkat yang sedang hingga berat. Skrining dan penapisan sindrom kardiometabolik menjadi salah satu upaya dalam mengidentifikasi adanya faktor risiko sindrom kardiometabolik di masyarakat. Identifikasi masyarakat dengan faktor risiko mengalami sindrom kardio-renal-metabolik melalui skrining kesehatan. Kegiatan dilakukan dalam rangkaian sosialisasi, pelatihan, edukasi, dan evaluasi. Karakteristik peserta yaitu mayoritas adalah perempuan, dan berusia paruh baya. Pada faktor risiko kesehatan metabolik terdapat 48 orang memiliki hipertensi dengan derajat yang bervariasi, terdapat 17 orang dengan kondisi pre-diabetes atau diabetes, terdapat 66 orang dengan asam urat normal, namun 30 orang memiliki asam urat yang tinggi, 75 orang dengan indeks massa tubuh berisiko dengan variasi berat badan berlebih hingga obesitas, 24 orang dengan hipertensi yang disertai pre-diabetes. Pada kesehatan ginjal terdapat 37 orang dengan proteinuria, dan  mayoritas peserta mengalami ketonuria. Monitoring faktor risiko secara berkala dan dalam jangka panjang dibutuhkan untuk melihat progresifitas faktor risiko. Tatalaksana dari faktor risiko yang ada di msyarakat disesuaikan dengan stadium sindrom KRM karena setiap stadium sindrom KRM memiliki fokus intervensi atau tatalaksana yang berbeda.

ABSTRACT

 

The burden of chronic disease results in the loss of almost 7 years of healthy life. The presence of disability conditions due to unmanaged and unidentified risk factors from the outset further increases the disease burden in Indonesia, especially for diseases within the cardio-renal-metabolic spectrum. This condition is also exacerbated by moderate to severe decline in kidney function. Screening and early detection for cardiometabolic syndrome serve as one effort to identify cardiometabolic syndrome risk factors in the community. To identify individuals in the community with risk factors for cardio-renal-metabolic syndrome through health screening. Activities were carried out in a series encompassing outreach, training, education, and evaluation. Participant characteristics showed a majority were female and middle-aged. Regarding metabolic health risk factors, 48 individuals had hypertension of varying degrees, 17 individuals had pre-diabetes or diabetes, 66 individuals had normal uric acid levels, but 30 individuals had high uric acid, 75 individuals had at-risk body mass index with variations from overweight to obesity, and 24 individuals had hypertension accompanied by pre-diabetes. Regarding kidney health, 37 individuals had proteinuria, and the majority of participants experienced ketonuria. Regular and long-term monitoring of risk factors is needed to monitor the progression of risk factors. Management of the risk factors present in the community should be adjusted according to the stages of the CRS syndrome, as each stage of CRS syndrome has different intervention or management focuses.
