Pahari-Stroke: Model Pemberdayaan Rumah Caregiver untuk Mewujudkan Rehabilitasi Pascastroke Berkelanjutan di Desa Antasan Senor Ilir

ABSTRACT
Inadequate continuity of post-stroke rehabilitation at the family level has contributed to an increased risk of disability, complications, and dependency among stroke survivors. Limited knowledge and skills of caregivers have remained a major barrier to the implementation of home-based rehabilitation in Antasan Senor Ilir Village. This community service program aimed to improve caregivers' knowledge and skills through the implementation of the PAHARI-STROKE Program as a Caregiver House empowerment model to support sustainable post-stroke rehabilitation. A participatory approach was implemented through health education, training, demonstrations, simulations, mentoring, home visits, monitoring, and evaluation involving 30 participants consisting of stroke survivors, family caregivers, PKK cadres, and community health volunteers. The program was conducted over ten meetings from June to July 2026. Program effectiveness was evaluated using pre-test, post-test, and observational skill checklists. Data were analyzed using descriptive statistics and a paired t-test. The mean caregiver knowledge score increased from 58.40 ± 9.82 to 87.60 ± 5.94 (p<0.001). Caregiver skills significantly improved in Range of Motion exercises, Activities of Daily Living, dietary management, medication adherence, and the utilization of the PAHARI-STROKE Mobile application. The program also established a Caregiver House as a community-based rehabilitation center. The PAHARI-STROKE Program effectively improved caregiver capacity and supported sustainable home-based post-stroke rehabilitation through community empowerment.

ABSTRAK
Rehabilitasi pascastroke yang tidak berkelanjutan di tingkat keluarga menyebabkan tingginya risiko kecacatan, komplikasi, dan ketergantungan penyintas stroke. Keterbatasan pengetahuan dan keterampilan caregiver menjadi salah satu kendala utama dalam pelaksanaan rehabilitasi berbasis rumah di Desa Antasan Senor Ilir. Kegiatan pengabdian kepada masyarakat ini bertujuan meningkatkan pengetahuan dan keterampilan caregiver melalui implementasi Program PAHARI-STROKE sebagai model pemberdayaan Rumah Caregiver untuk mendukung rehabilitasi pascastroke yang berkelanjutan. Kegiatan menggunakan pendekatan partisipatif melalui penyuluhan, pelatihan, demonstrasi, simulasi, pendampingan, home visit, monitoring, dan evaluasi terhadap 30 peserta yang terdiri atas penyintas pascastroke, caregiver keluarga, kader PKK, dan kader Posyandu. Program dilaksanakan selama 10 kali pertemuan pada Juni–Juli 2026. Evaluasi dilakukan menggunakan pre-test, post-test, lembar observasi keterampilan, dan dianalisis secara deskriptif serta uji paired t-test. Rata-rata skor pengetahuan meningkat dari 58,40 ± 9,82 menjadi 87,60 ± 5,94 (p<0,001). Keterampilan caregiver dalam melakukan rehabilitasi berbasis rumah juga meningkat pada seluruh indikator, termasuk latihan Range of Motion, Activity of Daily Living, pengelolaan diet, kepatuhan pengobatan, dan penggunaan aplikasi PAHARI-STROKE Mobile. Program juga menghasilkan pembentukan Rumah Caregiver sebagai pusat rehabilitasi berbasis komunitas. Program PAHARI-STROKE efektif meningkatkan kapasitas caregiver dan mendukung rehabilitasi pascastroke yang berkelanjutan melalui pemberdayaan masyarakat berbasis rumah.
