Profil Neuropati Perifer dan Korelasinya dengan Kadar Gula Darah Sewaktu di Panti Lansia Santa Anna

ABSTRACT

 

Diabetes, as a global disease, continues to increase worldwide without being limited by location. The burden of diabetes in the elderly population is a significant health issue. Peripheral neuropathy caused by diabetes is strongly associated with physical limitations in older individuals. This cross-sectional study aims to determine the characteristics of peripheral neuropathy in participants who meet the criteria, using a total sampling at Santa Anna on July 15, 2023, and explore its correlation with blood sugar levels. Evaluation of sensory and motor clinical symptoms using Neuropathy Symptom Score (NSS) questionnaire. Abnormal reflexes and sensory function were assessed using Neuropathy Deficit Score (NDS) questionnaire. Blood sugar levels were measured using POCT method following standard procedures. Statistical analysis using the Kruskall Wallis test. Out of the 68 participants, the average age was 74.06 years, with 67.6% of the participants being female. The overall average blood sugar level was found to be 115.82 mg/dL. Based on the NSS and NDS classifications, 8.8% and 23.5% of the total participants were categorized as having severe symptoms, respectively. The statistical analysis revealed a significant difference in mean blood sugar levels between the NSS groups (p-value: 0.003). However, no significant difference was observed in mean blood glucose levels between the NDS groups (p-value: 0.264). Clinically, the severe symptom group had a higher median blood glucose level compared to the normal group. It is noteworthy that elevated transient blood sugar levels tend to impact the subjective symptoms (NSS) of peripheral neuropathy earlier than the objective symptoms (NDS).

Sebagai penyakit yang tersebar di seluruh dunia, diabetes terus berkembang tanpa memandang tempat dengan jumlah pasien yang semakin meningkat. Beban diabetes pada populasi lanjut usia merupakan masalah kesehatan yang krusial. Neuropati perifer yang diakibatkan oleh diabetes dikaitkan dengan disabilitas fisik yang signifikan pada populasi lanjut usia. Penelitian potong lintang ini bertujuan untuk mengetahui profil neuropati perifer pada responden yang memenuhi kriteria dengan menggunakan total sampling di Panti Santa Anna pada 15 Juli 2023 serta korelasinya dengan kadar gula darah sewaktu. Evaluasi gejala klinis sensorik dan motorik menggunakan kuesioner Neuropathy Symptom Score (NSS). Refleks tidak normal dan penilaian sensorik dieevaluasi menggunakan kuesioner Neuropathy Deficit Score (NDS). Gula darah sewaktu diukur menggunakan POCT sesuai prosedur standar. Analisis statistik menggunakan uji Kruskall Wallis. Dari 68 responden, rata-rata usia adalah 74,06 tahun dengan 67,6% responden adalah perempuan. Dari seluruh responden, didapatkan rata-rata gula darah sewaktu sebesar 115,82 mg/dL. Berdasarkan NSS dan NDS, persentase responden yang termasuk dalam klasifikasi berat adalah sebesar 8,8% dan 23,5% dari keseluruhan responden. Hasil uji statistik Kruskall Wallis menyatakan adanya perbedaan rerata kadar gula darah sewaktu antar kelompok NSS (p-value : 0,003). Namun, didapatkan tidak adanya perbedaan rerata kadar gula darah sewaktu antar kelompok NDS (p-value : 0,264). Secara klinis, kelompok dalam kategori berat memiliki nilai median kadar gula darah sewaktu lebih tinggi dibanding kelompok normal. Kadar gula darah sewaktu yang tinggi cenderung mempengaruhi gejala subjektif (NSS) dari neuropati perifer terlebih dahulu dibandingkan dengan gejala objektif (NDS) dari neuropati perifer.
