ABSTRACT

 

The case-based payment system using INA-CBGs implemented in the National Health Insurance Program (JKN) aims to improve the efficiency of healthcare financing. However, in practice, discrepancies are still frequently found between BPJS Kesehatan claim tariffs and the actual costs of hospital services, commonly referred to as tariff discrepancies. One of the factors suspected to contribute to these discrepancies is Length of Stay (LOS), particularly in cardiovascular cases, which involve a high level of service complexity. This study aims to analyze the relationship between Length of Stay (LOS) and BPJS Kesehatan tariff discrepancies in inpatient claims for cardiovascular cases at Dr. Soedirman Kebumen Regional General Hospital. This study used a quantitative correlational study with a cross-sectional design and a total sampling method for 728 inpatient claims data from the study year. Data analysis was performed using univariate and bivariate analyses using Spearman's rho correlation tests. The analysis showed a strong and statistically significant relationship between Length of Stay (LOS) and BPJS Kesehatan tariff discrepancies (r = 0.695; p < 0.001), with a positive correlation direction. Length of Stay (LOS) has been shown to contribute significantly to cost discrepancies in inpatient claims, the longer the LOS, the greater the potential for BPJS Kesehatan tariff discrepancies in inpatient claims for cardiovascular cases. Hospitals need to improve the management of Length of Stay (LOS) to reduce tariff discrepancies

ABSTRAK

 

Sistem pembayaran berbasis INA-CBGs yang diterapkan dalam Program Jaminan Kesehatan Nasional (JKN) dirancang untuk meningkatkan efisiensi dalam pembiayaan pelayanan kesehatan. Namun dalam implementasinya masih sering ditemukan selisih antara tarif klaim BPJS Kesehatan dan biaya aktual pelayanan rumah sakit yang dikenal sebagai diskrepansi tarif. Salah satu faktor yang diduga berkontribusi terhadap diskrepansi tersebut adalah Length of Stay(LOS), terutama pada kasus kardiovaskular yang memiliki kompleksitas pelayanan tinggi. Penelitian ini bertujuan untuk menganalisis hubungan antara LOS dengan diskrepansi tarif BPJS Kesehatan pada klaim rawat inap kasus kardiovaskular di RSUD Dr. Soedirman Kebumen. Penelitian ini menggunakan jenis penelitian kuantitatif korelasional dengan rancangan pengambilan data cross-sectional dan metode pengambilan sampel total sampling terhadap 728 data klaim rawat inap periode Juli-Desember 2025. Analisis data dilakukan secara univariat dan bivariat menggunakan uji korelasi Spearman’s rho. Penelitian menunjukkan bahwa terdapat hubungan yang kuat dan signifikan antara LOSdan diskrepansi tarif BPJS Kesehatan (r = 0,695; p < 0,001), dengan arah korelasi positif. LOS terbukti berkontribusi secara signifikan terhadap terjadinya diskrepansi tarif pada klaim rawat inap, dimana semakin lama LOS, semakin besar potensi terjadinya diskrepansi tarif BPJS Kesehatan pada klaim rawat inap kasus kardiovaskular. Rumah sakit perlu meningkatkan pengelolaan LOS untuk mengurangi diskrepansi tarif.
