Chronic Kidney Disease Stadium V akibat Nefrosklerosis dengan Cardiorenal Syndrome pada Wanita 50 Tahun : Laporan Kasus
Abstract
Chronic Kidney Disease (CKD) stadium V merupakan tahap akhir penyakit ginjal yang sering disertai komplikasi kardiovaskular berat melalui mekanisme sindrom kardiorenal. Wanita 50 tahun dengan riwayat hipertensi 10 tahun dan kepatuhan obat buruk. Datang dengan sesak napas progresif, edema generalisata, dan mual. Tekanan darah 220/100 mmHg (hipertensi emergency), takipnea, saturasi oksigen menurun, peningkatan tekanan vena jugularis, edema perifer, dan kongesti paru. Anemia normositik normokromik, peningkatan ureum dan kreatinin, eGFR 5,48 mL/menit (CKD G5), hipokalemia. Hipertrofi ventrikel kiri. Kardiomegali, bendungan paru, dan efusi pleura bilateral. CKD G5 et causa nefrosklerosis dengan Congestive Heart Failure akibat hipertensi, disertai hipertensi emergency dan anemia penyakit kronik. Stabilisasi hemodinamik, terapi antihipertensi, diuretik, koreksi metabolik, terapi suportif, dan perencanaan hemodialisis. Kasus ini menunjukkan konsekuensi fatal dari hipertensi tidak terkontrol jangka panjang. Novelty kasus ini adalah manifestasi sindrom kardiorenal yang parah pada pasien usia relatif muda dengan kepatuhan pengobatan buruk, menekankan pentingnya deteksi dini CKD dan pengendalian tekanan darah untuk mencegah komplikasi multisistem mengancam jiwa.
